We provide home care services to help individuals with disabilities have the right support and assistance for them to stay independent and comfortable in their homes. Depending on the individual situation, we can arrange personal care services from a few hours a day to around-the-clock care.
Your physician should request Home Care before discharge from the hospital and determine your eligibility for Home Health Care services along with the Discharge Planner, Case Manager, or Community Agency. After receiving the referral, our intake Nurse will meet with you and your family to conduct an in-home assessment to review your home care needs, medications, and the supplies or equipment you will need. Your physician will be the one who will determine the type, frequency, and duration of the home visits. Along with your physician, we will develop an individualized care plan that will be changed periodically to ensure we provide the appropriate home health care services for your complex needs.
Upon admission into our Agency, we will assign a nurse coordinator for you who is responsible for integrating your services into the comprehensive plan of care. The Nursing Supervisor will visit your home from time to time to ensure that all the home health care services ordered by the physician are implemented by our staff properly.
